WWHA Documents

This area contains several documents that are used by the WWHA.

Most of the forms are in Adobe PDF format.  If needed Adobe Reader can be downloaded from the Adobe Website for free.

Documents needed for Registration:

  • USA Hockey Registration Receipt (non-WWHA form, also called IMR Form)
  • *WWHA Player Contract and Waiver of Liability
  • *USA Hockey Participant Code of Conduct (to be completed by the player)
  • *USA Hockey Concent to Treat / Medical History Form
  • *Parent/Legal Guardian Code of Conduct Agreement
